Based on visual observation the facility failed to assure the construction of the smoke barriers walls. The walls are required to be continuous and properly protected from penetrations and gaps. Unprotected penetrations would permit the movement of smoke from one compartment to the other in the facility. The deficient practice had the potential to affect 142 of 142 residents.

Findings:
During the facility tour, between the hours of 8:30 AM and 4: 30 PM on 12-14-11 it was observed that 1 ) Second floor above door in barrier wall CICU there is 1/2 inch hole with blue wires that needs to be sealed .2 ) Seal penetrations in Electrical Room and above DATA door around blue wires. 3 ) Seal above barrier door by staff lounge blue wires. 4 ) Seal around blue wires above barrier door by Director of Surgical Services. 5 ) East around blue wires by room 2117 above barrier door. 6 ) First floor East Wing seal above barrier door by room 1201 blue wires. 7 ) First Floor West Wing by Room 1217 above barrier door seal around blue wires.
