PHYSICAL ENVIRONMENT

Based on observations during facility tour, review of facility inspection records, and staff interviews, the facility failed to ensure that the hospital was constructed, arranged and maintained to ensure the safety of patients and staff.
Findings were:
Cross-refer to A-701 as it relates to failure of the facility to maintain all hospital systems and the physical plant; and
Cross-refer to A-710 as it relates to failure of the facility to meet the Life Safety Code Requirements of the National Fire Protection Association (NFPA).

MAINTENANCE OF PHYSICAL PLANT

Based on observations and staff interviews, the facility to ensure that the physical plant and overall hospital was maintained to ensure the safety and well-being of patients.
Findings were:

Cross-refer to Life Safety Code K-038 as it relates to failure of the facility to ensure that the required Exit access was arrange so that exits were readily accessible at all times.

LIFE SAFETY FROM FIRE

Based on observations and staff interviews, the facility failed to comply with all requirements of the Life Safety Code of the National Fire Protection Association.

Findings were:

Cross-refer to Life Safety Code K-020 as it relates to failure to ensure that the stairways and other vertical openings between floors are enclosed with construction having a fire resistance rating of at least one hour.

Cross-refer to Life Safety Code K-025 as it relates to the facility failure to ensure that Smoke barriers are constructed to provide at least one half hour fire resistance rating in accordance with 8.3.

Cross-refer to Life Safety Code K-029 as it relates to the facility failure to ensure that Hazardous areas were separated from other spaces by smoke resisting partitions and self-closing doors.

Cross-refer to Life Safety Code K-062 as it relates to the facility failure to ensure that automatic sprinkler systems are continuously maintained in reliable condition and are inspected and tested periodically.

Cross-refer to Life Safety Code K-070 as it relates to the facility failure to ensure that Portable space heating devices used in non-sleeping staff and employee areas had heating elements that did not exceed 212 degrees Fahrenheit (100 degrees centigrade) and that the units were listed for use in other than residential facilities.
