Based on observation and interview, the facility failed to maintain fire walls. The deficient practice affected 2 of 3 smoke compartments, staff and patients.
The findings include:
Observation and interview with the Site Manager of Engineering on 6/18/2019 at 2:10 PM confirmed improperly sealed penetrations in a 2-hour fire wall above ceiling in the physician's on-call room by 4 water pipes.

The Site Manager of Engineering was present when the deficiencies were identified and acknowledged by the Administrator during the exit conference on 6/18/2019.
