Based on document review and interview, the facility failed to maintain the proper category system for the use.
The medical air gas inspection from 10/14/2017 revealed the following:
1. Interview on 04/24/2019 with the chief nursing officer between 9:30 AM and 11:00 AM, revealed the medical air system has been made inoperable to staff due to a failed inspection.

2. Document review on 04/24/2019 between 9:30 AM and 11:00 AM, revealed the facility does not have two fully functioning compressors that eliminates oil anywhere in the compressor or provide a separation of the oil containing section.

3. Document review on 04/24/2019 between 9:30 AM and 11:00 AM, revealed the facility does not have two desiccant air dryers with filters and regulators for the medical air compressors.

4. Document review on 04/24/2019 between 9:30 AM and 11:00 AM, revealed the facility does not have dew point monitors for the medical air gas system.

5. Document review on 04/24/2019 between 9:30 AM and 11:00 AM, revealed the facility does not have carbon monoxide monitors for the medical air gas system.

The finding included:
Document review on 04/24/2019 at between 9:30 AM and 11:00 AM, revealed no medical air gas inspection was conducted in 2018.

Based on document review, the facility failed to maintain the emergency electrical system.
The finding included:
Document review on 04/24/2019 between 9:30 AM and 11:00 AM, revealed the engine control switch will not shut the emergency generator down and must be done manually. The maintenance staff on site stated they have to manually transfer the power back when local power is restored.
