Based on observation and testing, the facility failed to provide partitions that have a fire resistance rating of at least one hour.

Findings include:

While inspecting vertical openings on 06/29/10 through 6/30/10 the surveyor observed openings or un-sealed penetrations in the following locations in the following vertical shafts:

1) Vertical shaft located behind the service elevator shaft.
2) Vertical shaft located behind nurses station and across from main elevators.

These deficient practices have the potential of effecting the entire facility.

Based on observations the facility failed to provide the required ? hour fire resistance rating for smoke barrier walls.

While inspecting smoke barrier walls on 6/29/10 to 6/30/10 at appx 3:00 p.m., the surveyor and maintenance supervisor observed the smoke barrier wall located in the nurses lounge on the 2nd and 3rd floor was incomplete.

This deficient practice has the potential of affecting 2 of 4 smoke compartments on each floor. The administrator and maintenance director were notified during the survey and in the exit conference.
