Based on record review and staff interview, the facility failed to ensure that Patient #1, of one (1) of four (4) patients reviewed, had the right to formulate advance directives and to have hospital staff and practioners who provide care in the hospital to comply with these directives.
Findings include:
Record review for Patient #1 revealed no documented evidence of the patient's advance directives or code status.
Interview with the Risk Manager on 2/19/14 at 2:15 p.m. revealed that there was no policy and procedure in place to obtain the patients advance directive or code status. She stated, "We do not currently have a policy for that."
