PATIENT RIGHTS: CARE IN SAFE SETTING

Based on observation and interview, it was determined that the facility did not always maintain a safe environment for its patients.

Findings were:

Tour of patient rooms on 9/25/17 revealed inoperable mechanical beds with exposed wiring and non-functional electrical cords. The foot boards at the end of each bed had been removed, leaving exposed, sharp bars that could easily result in patient injury if a patient fell or slid onto them.

These findings were confirmed in interviews with the CEO and the DON that same day.

MAINTENANCE OF PHYSICAL PLANT

Based on observation and interview, it was determined that the facility was not always maintained to assure the safety of its patients and staff.

Findings were:

"OSHA/Bloodborne Pathogen Regulations Policy #138-030-060" stated in part "The facility provides sufficient housekeeping and maintenance personnel to maintain the interior and exterior of the facility in a safe, clean, orderly, and attractive manner."

Tour of the unit on 9/25/18 revealed the following:

* Inoperable mechanical patient beds were found with exposed wiring and non-functional electrical cords. The foot boards at the end of each bed had been removed, leaving exposed, sharp bars that could easily result in patient injury if a patient fell or slid onto them.
* Exposed plaster on walls
* Chipped and ill-fitting cabinetry
* A nonworking and dim fluorescent light in the patient hallway in front of the nurses' station
* Water damaged walls with base boards pulling from walls-particularly in the patient shower area.

These issues were confirmed with the Director of Nurses who toured with the surveyor on 9/25/18.
