Based on observation and interview the hospital failed to ensure the environment was maintained in such a manner as to ensure safety by having unlocked rooms that endanger patient safety. This has the potential to affect all patients in the hospital. Findings:
During a hospital tour on 05/18/10 at 2:00 p.m. with S2RN, VP Quality Improvement, and S4RN, Program Manager, the following observations were made:
The Laundry room door was unlocked. Inside the room was one 3 quart bottle of Clorox (nearly full) and one 3.93 pound box of Laundry detergent (powder).
The Lobby door was unlocked. There were members of a construction crew inside with tools scattered about, including a box cutter with blade exposed that was within reach of this surveyor.
The Staff Lounge door was unlocked. Inside was a freezer with electrical cord attached, microwave oven with electrical cord attached, and toaster with electrical cord attached. The bathroom was noted to have a shower head with 3' - 4' hose attached to it.
The physicians exam room was unlocked. There were two telephones on the desk with long phone cords, multiple computer cords, and an electrical cord for the computer. There was also a coffee pot with electrical cord in the room.
These findings were confirmed at the time of finding by S2RN, VP Quality Improvement, and S4RN, Program Manager. Both further confirmed that there was one patient identified as suicidal and two patients diagnosed with Dementia on the unit.
