Based on record review and interview, the facility failed to have an activities program (A 1537), a qualified individual to provide Social Services (A 1538), and resources to provide routine or emergency dental services (A 1551). Failing to have an activities program, social services and dental services result in the facility not meeting the requirements of Skilled Nursing Facility Services (A 1505). This failure also resulted in the condition of Swing Beds not to be met.

Findings:

Reference A 1505: Based on record review and interview, the facility failed confirmed with the skilled nursing facility requirements for Patient Activities, Social Services and Dental Services to two of two (#1 and #3) patients utilizing the swing beds.

Reference A 1537: Based on record review and interview, the facility failed to have for two of two (#1 and #3) patients utilizing the swing beds, an activities program directed by a qualified professional who is licensed or registered.

Reference A 1538: Based on record review and interview, the facility failed to have a qualified person to provide social services to two of two (#1 and #3) patients .

Reference A 1551: Based on record review and interview, the facility failed for two of two (#1 and #3) swing bed patients to have resources to provide routine or emergency dental services.

Findings:

Review of the facility's census revealed the facility has two patients, #1 and #3, utilizing the facility's swing beds.

Review of a list of all of the facility's staff members failed to reveal and Activities Person, Social Services staff or dental staff.

Interview with the Clinical Services Director on 09/06/12 at 2:25 PM revealed the facility does not have an activities person on staff or contracted and there is no activities program going on. "Staff just goes in and talks to the patients."

Interview with the Marketing/Swing Bed staff person on 9/7/12 at 10:01 AM revealed they do not have anyone hired or contracted to provide dental services. Further interview revealed he has been negotiating with a guy to obtain the services for the hospital, but does not have it finalized. He further stated that if it was needed he guess he would take them out to obtain the services.

Interview with the Marketing/Swing Bed person on 9/7/12 at 10:03 AM revealed there is no social service person contacted or on staff currently. Further interview revealed that he is currently acting as the social services person taking care of the seeing swing patients' discharge. Interview at the same date and time with the Clinical Services Director revealed that he is attempting to hire a Social Service staff person.
