Based on documentation review and document review, the facility failed to follow its policy, Customer Issue Resolution for 1 of 10 patients, PT4.  
1.  Review of policy Customer Issue Resolution, last approved 05/2015, indicated on page 4 of 7, K. "Concern/complaint details and resolutions are logged within the event reporting system". 
2.  Interview with P50 Director of Nursing on 11/27/17 at 2:15 pm, confirms that family called on 11/10/17 stating that facility gave patient too much Lithium which caused a hospitalization and wishes reimbursement for costs.  Grievance was not logged and letter to inform complainant of time frame for investigation not sent.
