Based on Observation, Interview and Record Review, the facility failed to provide a a safe setting that ensured protection from harm for 4 of 4 patients (#2, #3, #4, ) on Suicide Precautions as shown by the presence of potentially harmful objects observed in patients' rooms.

Record review of facility policy #200.32 titled Contraband and Restricted Articles, Patient Care Procedures dated 10/21/20 stated in part, that Houston Behavioral Healthcare Hospital does not permit any potentially harm-producing items, including sharp objects.
Observation on 11/16/20 at 10:15am-10:30am showed that adolescent female patient rooms #104 and #105 contained sharpened colored pencils. Room #104 had 9 pencils total and Room #105 had 2 pencils total.
In an interview on 11/16/20 at 10:30am, Staff # D stated that the pencils should not be present in any of the patient rooms and promptly removed the pencils from these rooms.
Record review on 11/16/20 at 12:30am showed that room #105 housed two female adolescents, patient #2 and #3, who were both currently on Suicide Precautions. Room #104 housed two patients, #4 and #5, who were also both on Suicide Precautions.
