PATIENT RIGHTS: RESTRAINT OR SECLUSION

Based on document review and interview the facility failed to ensure a physicians order was present for restraints for one (1) of ten (10) medical records(MR) reviewed (Patient # 10).

Findings include:


1. The hospital policy titled, "Use of Restraints and Seclusion", no policy number, indicated a restraint order must be obtained from an LIP (Licensed Independent Provider) immediately prior to or within 30 (thirty) minutes of initiating restraints or seclusion.

2. Review of patient # 10 medical record indicated Restraint/Seclusion review dated 6/6/23 indicated the initiation time for a restraint chair was 4:35 pm. The medical record indicated on 6/6/23 two Violent Restraint Child or Adolescent (9-17 years) orders were placed at 6:17 pm & 6:53 pm > 1.5 hours after initiation of restraint chair.

3. In interview on 6/23/23 at approximately at 10:00 am with administrative staff member A # 9 (Interim Medical Director), confirmed that there should have been a restraint order in place immediately prior to or 30 minutes after initiating restraints of patient # 10.
