Based on observation and interview, the facility failed to follow the standards established to prevent the spread of fire, smoke and the products of combustion from hazardous areas to the part of the building occupied by patients, staff and the public. Per NFPA 101 Chapter 18.3.2.1 ' Any hazardous area shall be protected in accordance with Section 8.4. The areas described in Table 18.3.2.1 shall be protected as indicated. ' Per Table 18.3.2.1, ' Trash collection rooms-1 hour ' And, per 8.4.1.3 ' Doors in barriers required to have a fire restive rating shall have a ? hour fire protection rating and shall be self-closing or automatic-closing in accordance with 7.2.1.8. '
Findings include:
During the tour of the building on June 10, 2013 at approximately 11:15 a.m., the door to the bio-hazard trash storage room was tested and it failed to close fully and engage the locking mechanism. The door made contact with the door frame and would close only when forced. Inquiry of the maintenance director revealed that the door was subject to damage from the movement of large trash containers.
These finding were re-confirmed with the administrator during the exit conference at 3:00 p.m. on June 10, 2013.

Based on observations and interviews with facility staff, the facility did not maintain the following per the National Electric Code (NEC), Article 517 and NFPA 99. The facility failed to maintain fire protection and occupancy features necessary to minimize hazards. Per Chapter 9.1.2 LSC 2000. ' Electrical wiring and equipment shall be in accordance with NFPA 70, ' Also, per NFPA 99, 9-2.1.2.2 ... ' Material and gauge...of electrical wiring shall be a type suitable for the particular application. '
Findings include:
Observations and interview of the maintenance assistant during the life safety tour on June 10, 2013 revealed that the following electrical applications were not in accordance with NFPA 70, the National Electrical Code:
At approximately 11:45 a.m. in the Material Management office, a power strip was observed to connect a microwave oven and a refrigerator to the wall outlet.
At approximately 11:55 a.m. electrical panel marked as " CKP1 " spares were indicated on the circuit schedule that were observed to be in the on position. There were several inaccuracies noted on the panel board.
At approximately 12:15 p.m., in the dining room the serving area was observed to have two crockpots connected with an extension cord and a power strip connected in series.
These finding were re-confirmed with the administrator during the exit conference at 3:00 p.m. on June 10, 2013.
