Based on observation and staff interviews the facility failed to maintain portable space heaters in accordance with National Fire Protection Association (NFPA) 101. This deficient practice could affect all residents, staff and visitors in the areas referenced. The facility's census was 7.

Findings include:

1. A facility tour conducted on 9/29/20 at 11:11 a.m. revealed a portable space heater under a desk in the reception office.

2. An interview on 09/29/20 at approximately 2:55 p.m. with the Facilities Director verified these findings. The findings were were also acknowledged by the Administrator at the exit interview on 09/29/20.

1. A facility tour on 09/29/20 at 9:29 a.m. revealed a fan and a treadmill plugged into a power strip at the Bruceton Mills Physical Therapy location.

2. A facility tour on 09/29/20 at 9:31 a.m. revealed a coffee maker plugged into a power strip at the Bruceton Mills Physical Therapy location.

3. A facility tour on 09/29/20 at 11:05 a.m. revealed a microwave plugged into a power strip in the Administration office break room.

4. An interview on 09/29/20 at approximately 2:55 p.m. with the facility's Maintenance Supervisor verified these findings. The findings were were also acknowledged by the Administrator at the exit interview on 09/29/20.
