Based on observation and interview, the facility failed to maintain the corridor doors. This was evidenced by a corridor door that failed to latch. This affected one of three floors and could result in the delay of evacuating during an emergency.

Findings:

During a tour of the facilities and interview with staff on 8/6/19, the corridor doors were observed.

1. At 9:26 a.m., the door to Resident Room 634 failed to latch when tested. The door was equip with a self-closer and was tested three times by staff and failed to latch each time. Upon interview, Staff 1 confirmed the finding.
