a) Exhaust fans were not on a preventive maintenance program. Exhaust fans in patient rooms, decontamination, and central sterilization rooms were not working.

1) Based on observation and interview with staff, the facility failed to have a Type I Essential Electrical System powered by a generator with a transfer switch and separate power supply. The EES is in accordance with NFPA 99, 3.4.2.2, 3.4.2.1.4. Findings include:
a) The receptacle supplying the Blood Bank was not marked in red and could not be verified that the receptacles were on the emergency power supply system.

Based on observation and interview with staff, the facility failed to inspect the Generator weekly and exercise under load for 30 minutes per month in accordance with NFPA 99. 3.4.4.1. Findings include:
a) The emergency log was not completely filled out. There was no documentation for Oil Temperature and the Fuel Level was not logged.

a) Receptacles located in patient care areas have not had a preventive maintenance check that included Receptacle Testing in Patient Care Areas (1) The physical integrity of each receptacle shall be con-
firmed by visual inspection. (2) The continuity of the grounding circuit in each electrical receptacle shall be verified (3) Correct polarity of the hot and neutral connections in each electrical receptacle shall be confirmed (4) The retention force of the grounding blade of each electrical receptacle (except locking-type receptacles) shall be not less than 115 g (4 oz).
