Based on record review and interview, the facility staff failed to ensure PIV (Peripheral Intravenous) access established outside of hospital was changed per policy in 1 of 11 Patients (#2) closed medical records reviewed in a total universe of 11 medical records reviewed.
Findings include:
A review of facility policy titled, "Intravenous Therapy, Peripheral", Number: 69812 last revision date 06/08/2022 revealed, "G. In adults, replace a PIVC inserted under known or suspected suboptimal aseptic technique (i.e., in the field or under emergent conditions), when patient is stabilized 24-48 hours."
A review of Patient #2's medical record was completed. Nursing flow sheet revealed a peripheral IV was placed by EMS on the Left antecubital area on 07/21/2022 at 10:00 AM. IV site was removed on 07/25/2022 at 12:55 PM prior to discharge from the facility.
During an interview on 03/22/2023 at 10:30 AM, the medical record review findings were discussed with and confirmed by Clinical Informatics R, who stated It does not appear that the IV site was changed.
