Based on document reviews and interview, the facility failed to ensure annual restraint training was completed for 3 of 5 Providers (Provider #1 and Provider #2).
Findings:
The 'Use of Restraints' policy, approved 2/2019, indicated that staff involved with the application of a restraint, implementation of seclusion, providing care for a patient in restraint or seclusion, or with assessing and monitoring the condition of the restrained or secluded patient, will receive training annually.
Provider #1 and #2 had annual restraint on 2/15/19. As of 3/11/20, there was no evidence of the annual restraint training having been completed.
On 3/11/20 at 3:00 P.M., the Director of Accreditation and Regulatory Affairs acknowledged that tese two providers were required to complete annual training.
