COVID-19 Vaccination of Facility Staff

Based on review of facility policies, review of employees' vaccination records, and interviews, the facility failed to develop and implement policies and procedures to ensure 3 employees out of 15 employees reviewed were fully vaccinated for COVID-19. The facility also failed to develop and implement policies and procedures for employees to apply for and obtain exemptions from COVID-19 vaccinations. The facility also failed to develop and implement policies and procedures for employees to notify the facility of their vaccination status.
The findings included:
Review of facility policy titled, "COVID-19 Worksite Safety Measures and Expectations for State Facilities" effective date 4/1/2022 revealed there was no requirement for facility employees to be vaccinated. Continued review revealed "...Employees encouraged to share vaccination status with the Department..." but there was no requirement for staff to notify the facility of their vaccination status. There was no policy or procedure for employees to apply for exemptions from COVID-19 vaccinations and no policy requiring staff be vaccinated for COVID-19.
Review of 15 random personnel files of facility employees revealed:
12 employees fully vaccinated for COVID-19.
2 employees had only one vaccination for COVID-19.
1 employee had no vaccinations for COVID-19.
With 3 employees out of 15 reviewed not being fully vaccinated for COVID-19 it was determined:
80% of employees sampled were fully vaccinated for COVID-19.
20% of employees sampled were not fully vaccinated for COVID-19.
Interview in the Administration Conference Room with the facility's Superintendent on 5/17/2022 at 10:00 AM confirmed the facility did not have a policy requiring staff be vaccinated for COVID-19.
Interview in the Administration Conference Room with the facility's Assistant Superintendent on 5/24/2022 at 11:30 AM revealed the facility did not have a policy requiring staff to share their vaccination status with the facility. Interview revealed the facility obtained the employees' vaccination status from the state's vaccine data website. Interview revealed the facility did not have a policy or procedure for employee exemptions from COVID-19 vaccinations.
