PATIENT RIGHTS: RESTRAINT OR SECLUSION

Based on record review and staff interview, the facility failed to ensure the timely renewal of restraint orders for 1 of 11 patients (patient #9) reviewed during the survey.
Findings:
-Review of the medical record on 6/17/2020 for patient #9 reveals that the initial physician order for restraints was written on 5/28/2020 at 9:05 AM. The patient was in restraints from 9:00 AM to 6:30 PM (approximately 9.5 hours) without staff obtaining every four hour renewal orders to continue restraint.

-A 6/17/2020 review of facility policy # 7310 entitled "Patient Restraints Medical Violent and Seclusion", revised October 2019, indicates that restraint orders for patients age 18 and older are limited to 4 hours, after which time the order must be renewed and a face-to-face assessment conducted.

-The Director of Patient Safety confirmed on interview on 5/21/2021 that renewal orders for restraint were not present in the patient medical record.
