Based on observations, review of the pest control contract, detailed invoices from June to August for pest control and interviews with patients and staff it was determined the facility failed to ensure a clean and sanitary environment and pest control was maintained. This had the potential to negatively affect all patients admitted to the facility.
Findings include:
1. On 10/8/19 at 8:26 AM a tour of the facility was conducted with the Employee Identifier (EI) # 1, Director of Nursing (DON).
During the tour at approximately 8:50 AM the medical surgical unit was observed. The surveyors inspected occupied and unoccupied patient rooms.
At 8:52 AM, upon observation of Room 118, the surveyor observed a large roach behind the door on the baseboard of the wall. The roach was still alive at the time of the tour. The surveyor also observed a black chair torn in several areas in the corner of the room that was unclean and full of dust. Further observation of the restroom connected to the room revealed several dead black bugs on the window seal, on the floor and under the toilet. The surveyor observed a form on the back of the restroom door and documentation on that form revealed the restroom was last cleaned on 8/21/19.
The surveyor presented the findings to EI # 1 at the time of the tour. EI #1 informed the surveyor that "due to the drop in census" there had not been a patient in the room (Room 118) for "several months" and that housekeeping "typically don't clean if there's nothing to clean" in rooms that have not been "occupied in a while".
An interview was conducted 10/8/19 at 9:06 AM, EI # 1 informed the surveyor that he/she was not sure how often the unoccupied rooms were cleaned and confirmed the above mentioned findings in Room 118.

2. An interview was conducted on 10/8/19 at 9:20 AM with the patient and the visitor.
During the interview with the patient about the safety and cleanliness of the room the patient stated "house keeping has come in and cleaned the days I have been here and I do not see any problems."
After speaking with the patient the visitor spoke up and said "you should have been here last night (10/7/19). I was sitting here and all the sudden a big roach ran across the room. I tried to kill it but it was way to fast and got away."
Observations of all unoccupied patient rooms and several occupied rooms revealed on the bathroom doors revealed a form with the title of "This restroom was cleaned by:" and had signatures of each house keeping employee who was responsible for cleaning the bathroom.
Further observations of the patient rooms revealed no documentation in the patient room or in the housekeeping supervisors office the patient rooms had been cleaned.
An interview was conducted with EI # 1 at 9:30 AM after the observations and interviews with the patients and/or the visitor. EI # 1 confirmed the above mentioned findings.
3. A tour of the CDU (Chemical Dependant Unit) was conducted on 10/8/19 at 9:40 AM with EI # 1.
During the tour of the CDU area the surveyor entered room # 158, turned on lights and discovered the lights did not work.
The surveyor asked EI # 1 what was wrong with the lights and EI # 1 turned on both light switches and the lights failed to come on.
EI # 1 informed the manager of the CDU area about the lights and maintenance was to be notified.
An interview was conducted with EI # 1 at 9:45 AM who confirmed the above mentioned findings.
4. Review of the pest control contract and Detailed Service Reports from the pest control company on 10/9/19 at 11:30 AM for the months of April 23,2019, June 27, 2019 and July 29, 2019 revealed no documentation the patient rooms on the medical/Surgical area had been sprayed or bated for roach control.
An interview was conducted with EI # 1 on 10/9/19 at 12:00 PM who confirmed the above mentioned findings.
