Based on tour and observations conducted in the PES (Psychiatric Emergency Services) area it was determined that the areas used for patient care were not maintained in a sanitary manner. The census at the time of the tour was 11.

Findings include:

The tour of the PES unit was conducted on 04/20/11 at 2:30 PM with Staff F and G. The supply cart located outside of the nurses station that held supplies used for patient testing contained dust inside the bottom of all drawers. The floor between this supply cabinet and the wall had a thick build up of black dirt and dust. The shelves in the medication room had a thick build up of dust and the locked medication refrigerator had a black substance on the bottom of the inside of the refrigerator and on the shelves inside the door. The paper towel and soap dispenser had a large build up of dust on the top. Bed 3 had brown and red stains on the wall behind the cot. The bottom of this cot in Bed 3 had a large build up of a black substance and a brown stain along the side of the cot at the foot release. The corners of the room along the floor had a large build up of dirt and dust. Bed 4 had brown and red stains on the wall behind the cot and a build up of dirt along the window sill of the door.

The plastic chairs located both inside Bed 1,2 and 3 and in the hallway outside of these rooms had a large build up of a black substance.

An observation was made at 2:40 PM of a PES staff member cleaning the cots to prepare them for the next patient. This staff member used the same rag to clean 3 cots. After the rag was sprayed with a cleaning solution the surface of cots were completely wiped down from top to bottom and the dirty rag was not discarded between cots.

All findings were shared with Staff F and G on 04/20/11 at 2:44 PM.
