Based on observation and interview the facility failed to assure that the fire doors are latching properly. This deficient practice may cause fire/smoke to travel to adjacent stairway or building. This will affect 2 out of 11 smoke zones in the building. This facility has a capacity of 25 with a census of 5 residents at the time of the survey.

FINDINGS INCLUDE:

During the tour on 6/14/11 from 9 AM to 4 PM it was found that the 3 hour fire rated corridor door by the laboratory and stairway did not latch when released from the magnetic holding device.

Staff "M3" and "HM1" were present and aware of the finding.

NFPA Standard: Health care occupancies in buildings housing other occupancies shall be completely separated from them by construction having a fire resistance rating of not less than 2 hours as provided for additions in 18/19.1.1.4. 2000 NFPA 101, 18/19.1.2.3

Based on observation, record review and interview the facility failed to exercise the generator at 30% of its load capacity monthly. This may cause a failure of the generator system which may leave the entire building without power. This will affect all 11 smoke zones in the building. This facility has a capacity 25 with a census of 5 residents at the time of the survey.

FINDINGS INCLUDE:

During the tour on 6/14/11 from 9 AM to 4 PM it was found that the generator was not being tested at 30% of the 1561 amp load capacity which equals 468 amps minimum each month.

Staff "M3" and "HM1" were present and aware of the finding.

NFPA Standard: Level 1 and level 2 EPSSs, including all appurtenant components, shall be inspected weekly and shall be exercised under load monthly for a minimum of 30 minutes. 1999 NFPA 110, 6.4.1 and 6.4.2

Based on observation and interview the facility failed to assure that electrical outlets are GFCI when located next to a water source and that extension cords are not used for wiring. This deficient practice may cause a fire/smoke to occur in 3 out of 11 smoke zones. This facility has a capacity of 25 with a census of 5 residents at the time of the survey.

FINDINGS INCLUDE:

During the tour on 6/14/11 from 9 AM to 4 PM the following items were found:

1.) Rooms 308, 288, and 101 have wash sinks with electrical outlets within 6 feet that are not GFCI.
2.) The 3rd floor respiratory exercise room has an extension cord used for the culligan water cooler.

NFPA Standard: All 125-volt, single-phase, 15- and 20-ampere receptacles installed in bathrooms, garages, storage areas, work areas, and area of similar use shall have Ground-Fault Circuit-Interrupter (GFCI) protection. 1999 NFPA 70, 210-8.

NFPA Standard: Temporary electrical power and lighting installations shall be permitted (600 volts or less): (a) During Construction - temporary electric power and lighting installations shall be permitted during the period of construction, remodeling, maintenance, repair, or demolition of buildings, structures, equipment, or similar activities; (b) 90 Days - temporary electrical power and lighting installations shall be permitted for a period not to exceed 90 days for Christmas decorative lighting and similar purposes; (c) Emergencies and Tests - temporary electrical power and lighting installations shall be permitted during emergencies and for tests, experiments, and developmental work.; (d) Removal - temporary wiring shall be removed immediately upon completion of construction or purpose for which the wiring was installed. 1999 NFPA 70, 3-1.2.1.
