Based on observations performed on the follow up visit, it was determined that the facility failed to maintain patients safe and accident-free when not responding at the time of being called by the nursing calls. This affected 2 patients of 7 patients admitted in the emergency room.

Findings include: New Deficiency

During the observations performed on 7/25/17 at 9:15 am on the emergency room area for the follow up visit, it was observed on the new IV room medication room 2 patients with Intravenous medication watching TV. Surveyor asked to the patients if they have the necessity to use the call system to asked get help from the nurses and they answered no.

Surveyor at 9:16 am push the bottom of one of the call system and wait for the response of the nurses. At 9:17 am one of the nurse answered the call from the nurse station. The following was the answer from the nurse "Hola", "Buenas", "Hola", "Diga", "Diga". The surveyor pushed again the bottom about 30 seconds after the first call. At 9:18 am the same nurse answered the call from the nurse station. Again she said: "Buenas", "Hola", "Hola", "Buenas". After 3 minutes and 25 seconds the nurse arrived to the area to ask if something is wrong.

On 5/10/17 at 9:15 am during the validation survey this room was visited, an Immediate Jeopardy was called because this room lacks of the call system. This room is far from the nursing station.
