Based on observation and staff interview, the sprinkler heads have not been maintained as required, making the sprinkler heads not operate properly, affecting all residents and occupants in 1 of 6 smoke zones. The facility has a capacity of 25 with a census of 12.
FINDINGS INCLUDE:
During the survey conducted on 05/29/13 between 9:00 A.M. and 3:00 P.M., the following is observed.
1.) Surgery waiting room has a painted sprinkler head near the entrance.
2.) EMS station room has two loaded sprinkler heads.
3.) Linen closet located in O.B. area has a corroded sprinkler head.
Maintenance staff A was present and acknowledged the findings.
NFPA Standard: Required automatic sprinkler systems are continuously maintained in reliable operating condition and are inspected and tested periodically. 18.7.6, 19.7.6, 4.6.12, NFPA 13, NFPA 25 9.7.5.

Based on observation and staff interview, the facility failed to ensure that the cooking equipment was put back in the correct location under the suppression nozzles. If a fire was to occur the nozzles would not be able to suppress the fire. This affects all residents and occupants in 1 of 6 smoke zones. This facility has a capacity of 25 with a census of 12.

During the survey conducted on 05/29/13 between 9:00 A.M. and 3:00 P.M., the following is observed.
1.) The kitchen staff failed to maintain the locations of the cooking equipment, to ensure that the suppression system nozzles are located in the correct position above the equipment.
Maintenance staff A was present and acknowledged the finding.
NFPA Standard: Cooking facilities shall be protected in accordance with 9.2.3, 18.3.2.6, 19.3.2.6, NFPA 96.
