Based on Observation and interview with facility staff, the facility failed to maintain smoke barriers in the facility. Failure to maintain the smoke barrier puts all patients, staff and visitors in the facility at risk during a fire due to the spread of smoke. Census was 0.
Findings are:
1. On 09-08-14 at 10:32 A.M., observation reveled 8 feet X 2 Inch void at the top of the wall and small voids around the pipe penetrations in the smoke barrier above the separation doors leading into X-Ray wing.
Interview with Maintenance " A " at 10:32 AM verified there are voids in the smoke barrier.
2. On 09-08-14 at 10:45 AM., observation reveled an approximate 4 feet X 1 foot void in the smoke barrier of the East wall in the postage metering office.
Interview with Maintenance " A " at 10:45 AM verified the void in the wall.

Failure to maintain these hazardous areas smoke tight puts all the residents of the facility patient care area at risk should a fire develop in the hazard areas because of the spread of smoke. Census was 0.
Findings are:
1. On 09-08-14 at 1:45 PM, observation reveled that there was not an automatic door closure on the storage room by the chapel.
Interview at 1:45 PM with Maintenance " A " confirmed the lack of the door closure.
