Based on observations and maintenance staff interviews on 5/29/13 during an onsite revist, the facility failed to maintain minimum Standards of the 2000 Edition of the National Fire Protection Association's (NFPA).

Based on observations and maintenance staff interviews on 5/29/13 during an onsite revisit, the facility failed to maintain minimum Standards of the 2000 Edition of the National Fire Protection Association's (NFPA) 101 Life Safety Code.

Findings include:

1) K054 failure of replacement of or calibration of 11 smoke detectors throughout the facility, and

2) K062 failure to repair to the back flow preventor for the fire sprinkler system after testing identified parts were needed.
