Based on record review and staff interview during a complaint investigation on 10-9-2019, the facility failed to follow the Emergency Plan for Operating Room Fires. This condition created confusion and delayed fire department response to a fire emergency during a surgical procedure.
Findings are:
Record review and staff interview on 10-9-2019 between 9:00 AM and 12:00 PM revealed the following:
Statements made by facility staff and facility staff report show that the fire alarm activation was not initiated by the Circulating RN as required in the facilities Emergency Plan for Operating Room Fires. Fire alarm was activated 15 minutes after the fire by maintenance/security staff.
