Based on interview and record review it was determined the facility failed to refer a complaint regarding the quality of care and premature discharge of Patient 5 to the appropriate quality improvement process as required. Findings include:
During the survey it was revealed that on 8/18/10 the hospital received a complaint regarding the care that Patient 5 had received during an 8/14/10 visit to the Emergency Department. That complaint was registered with the hospital Patient Advocate, and included concerns regarding the quality of care and premature discharge of Patient 5.
Interveiw with Witnesses A, B and C on 10/12/10 determined that those concerns had not been referred to the appropriate hospital utilization and quality improvement process. At the time of the survey no such referral had been made.
The facility failed to ensure that concerns regarding the quality of care and premature discharge of Patient 5 were referred for appropriate investigation and quality review as required.
