Based on observation and interview, the facility failed to ensure monthly quick checks were performed at all fire extinguishers installed throughout the facility, as referenced by NFPA 101, 2000, 9.7.4.1 and required by NFPA 10 19.3.5.6.

Observations during the afternoon of 02/23/16, revealed the tag installed on fire extinguishers indicated an installation date of December 2015. There was no indication that the required monthly inspections were performed on the fire extinguishers during January 2016.

In an interview during the afternoon of 02/23/16, the Director of Plant Operations stated that he would perform the fire extinguisher monthly quick check for February 2016 and the following months.

Based on observation and interview, the facility failed to ensure all electrical receptacles located within three feet of a water source were Ground Fault Circuit Interrupter (GFCI), as required by NFPA 70, §§517.20 and 517.21.

Observations during the afternoon of 02/23/16, revealed the electrical receptacles installed within three feet of a water source (sink at Men's Restroom at Waiting Area) was not a Ground Fault Circuit Interrupter (GFCI).

In an interview during the afternoon of 02/23/16, the Director of Plant Operations stated that he would replace the electrical receptacle in the Men's Restroom with a GFCI and verify that all other electrical receptacle located within three feet of a water source are GFCI or they will be replaced also.
