Based on observation and staff interview, the facility failed to maintain fire walls at eight locations in the facility.


Findings were:

On facility tour between 02:00 p.m. and 04:00 p.m. on 02/02/10, observation revealed the following violations:

(a) Penetrations through rated fire wall at staff locker room.

(b) Penetrations through rated fire wall across from staff locker room.

(c) Network over fill sleeve through rated fire wall over fire doors next to staff and physician break room.

(d) Penetrations through rated fire wall over fire doors in surgery area.

(e) Penetrations through rated fire wall in main laboratory.

(f) Open sleeve in fire wall next to elevator equipment room.

(g) Penetrations through rated fire wall over fire doors in radiology.

(h) Penetration through rated fire wall in radiology waiting room.

(i) Fire walls were not labeled.

In an interview conducted at the time of observation between 2:00 p.m. and 4:00 p.m. on 02/02/10, staff member M stated that all fire walls were sealed during final inspection. The contractor had used puddy patch to seal the penetrations and the heat in the attic space caused the patch to fall off the penetrations. The staff member stated that all penetrations would be sealed.
