Based on clinical record review, it was determined the Facility failed to ensure 3 (#4, #5 and #19) of 17 (#2-#7, #9-#16, #18, #19 and #21) Medicare patients did not have signed Medicare Important Notices in the clinical record. Failure to ensure issuance of the Medicare Important Notice did not allow the patients the opportunity to be informed of and aware of their rights as a hospital patient on the day of survey. Findings follow:
A. Review of the clinical records of Patient #4 and #19 revealed unsigned Medicare Important Notices in the clinical record. Finding verified by the Health Information Manager Discharge Supervisor at 1315 on 11-08-11.
B. Review of the clinical record of Patient #5 revealed there was no Medicare Important Notice in the clinical record. Findings verified by the Health Information Manager Discharge Supervisor at 1315 on 11-08-11.
