Based on observation and staff interview, the facility failed to provide means of egress that are maintained free of all obstructions or impediments to a full instant use in case of fire or other emergency. The deficient practice would prevent these exits from being arranged so that they are readily available and accessible, affecting 0 patients and all occupants in 1 of 13 smoke zones. This facility has a capacity of 58 and a census of 29.

Findings Include:

On 11/19/2013 between 10:00 AM and 4:00 PM the following is observed:

1. At 3:04 P.M. it is discovered that the exit corridor outside main IT room is obstructed with beds and wheelchairs.

The Director of Environmental Services was present and acknowledged the findings.

Exit access is arranged so that exits are readily accessible at all times in accordance with section 7.1. 18.2.1

Based on observation and staff interview, the facility did not ensure that electrical wiring and equipment is installed and maintained in accordance with NFPA 70, National Electrical Code. This deficient practice does not ensure prevention of an electrical fire or electric shock hazard, affecting 4 of 13 smoke zones. This facility has a capacity of 58 and a census of 29.

Findings Include:

On 11/19/2013 between 10:00 AM and 4:00 PM the following is observed:

1. At 1:45 P.M. it is discovered that an extension cord is used as permanent wiring in the business office.

2. At 1:53 P.M. it is discovered that a suspended power strip is in the ICU check-in desk area.

3. At 1:50 P.M. it is discovered that power strips are linked together in the ICU nurse desk.

4. At 2:05 P.M. it is discovered that an extension cord is being used as permanent wiring located in the pharmacy.

5. At 2:30 P.M. it is discovered that an extension cord is being used as permanent wiring in the IT office.

The Director of Environmental Services was present and acknowledged the findings.

Electrical wiring and equipment is in accordance with NFPA 70, National Electrical Code. 9.1.2
