Based on visual observation the facility failed to provide exit lighting that was illuminated along the entire length of the exit access. Exit lights provide visual assurance of the direction of the exit door. This deficient practice has the potential to affect all of the occupants in the building..

Findings:

During the facility tour, on 12-28-15 between the hours of 9:30 a.m. and 11:00 a.m. the exit signs located in Sections A, C, and D were observed with no illumination.

Interview with maintenance personnel revealed the facility was not aware the signs were not illuminated.

The devices installed for the system's activation shall be by manual, detection or extinguishing operation which gives a sense of security to offer an advance warning in an emergency. This deficiency could potentially affect all of the occupants of the building.

Based on visual observation the facility failed to assure that the complete, supervised, automatic sprinkler system was inspected and tested in accordance with the requirements of NFPA 25. Activation of the sprinkler system shall trigger notification of the emergency to the fire alarm system within 90 seconds, which results in protection of life and property. This deficiency has the potential to affect occupants that may enter the building.

Findings:

During the facility tour on 12-29-2015 between the hours of 1:00 p.m. and 2:00 p.m. review of the sprinkler inspection documentation revealed the system was overdue for the required annual and quarterly inspections. The last inspection date was 11-29-2011.
