Based on medical record review, policy review and staff interview the facility failed to complete EKG monitoring per physician orders. This affected one (Patient #1) of ten medical records reviewed. The active census was 35.
Findings include:
Review of the Policy and Procedure Number CS.114 ( Effective date 05/14/18) for Physician Orders states the Registered Nurse coordinates the implementation of all physician orders and utilizes the most recent orders written by the physician.
Review of physician documentation dated 05/06/19 noted Patient #1 was given two injections of the psychotropic medication (Invega) and an accidental and/or unintentional overdose occurred. The physician ordered EKG monitoring twice daily per the poison control recommendations.
The medical record lacked evidence the EKG's were completed per physician orders. This finding was confirmed with Staff B on 05/29/19 at 9:45 AM.
