Based on personnel review and staff interview the facility failed to follow the policies and procedures of the hospital by not ensuring that nursing staff had completed the required crisis prevention intervention training for one (1) of five (5) staff reviewed (Staff 6). This failure has to potential to affect all patients receiving care at the hospital.
Findings:
A review of personnel record for staff 6 revealed that staff 6 failed to complete required Crisis Prevention Intervention training.
Review of the job description for a "Registered Nurse," under the section of Licenses and Certifications states, "De Escalation Training within six (6) months of hire is required."
An interview was conducted on 7/23/25 at 4:30 p.m. with staff 9. Staff 9 stated, "Crisis Prevention Intervention is a requirement for a registered nurse. You have to have it by six (6) months after hire, then update regularly. Staff 6 doesn't have that training."
