Based on medical record review and interview, it was determined nursing services failed to ensure treatments were administered as ordered for 1 of 3 (Patient #2) sampled patients.
The findings included:
1. Medical record review revealed Patient #2 was admitted to the hospital rehab unit on 3/20/15 with diagnoses of Encephalopathy, Mental Status Change, Lower Limb Weakness, Hypertension, Renal Failure, Hypotension and Bradycardia.
Review of the 3/20/15 physician's orders revealed an order for Knee High Thromboembolic Disease (TED) hose, apply while out of bed and remove at bedtime.
Review of the 3/20/5 nursing assessment revealed the patient had the TED hose.
There was no other documentation during the 3/20/15 - 3/24/15 Rehab Unit admission the patient wore the TED hose when up out of the bed.
In an interview on 8/11/15 at 1:15 PM in the conference room the Assistant Chief Nursing Officer verified there was no other documentation the patient had the TED on while out of bed.
