Based on record review and interview, the provider failed to conduct quarterly fire drills each quarter for the three shifts during two of the four previous quarters for the twelve month period beginning January 2015. Findings include:
1. Fire drill record review revealed no documentation indicating a fire drill was conducted for: the first shift (7 a.m. to 2 p.m.) during the last two quarters of 2015 (August through December). Exit interview with the administrator at 2:00 p.m. 1/12/16 revealed he was unaware fire drills were not held for those identified shifts each quarter.
