Based on observation, review of policy and interview, it was determined the facility failed to ensure outdated medications were not available for patient use in one of one crash cart for 1 East.
By not disposing of expired medications, the facility could not assure the safety and efficacy of the product past the manufacture's expiration date. The failed practice had the potential to affect all patients admitted for treatment. Findings Follow:
A. A tour of 1 East on 05/13/13 from 1220 to 1340 revealed expired medications in the crash cart as follows:
1) One Furosemide 10 mg (milligrams)/ ml (milliliter), 4 ml vial for injection expired 05-01-13; and
2) One Procainamide 1 gram/ 10 ml vial for injection expired 05-01-13.
B. Review of policy titled " Expiration Dates of Medication (Pharmacy) " stated " All expired, damaged, and/or contaminated medications are removed from pharmacy stock, Omnicell inventory, and floor stock prior to their expiration date. "
C. Findings were verified, through interview, with the Executive Director of Inpatient Services on 05-13-13 at 1237.
