Based on interview and record review, the hospital did not accurately write and/or spell the name 1 of 1 patient (Patient #1) who presented in the emergency department (ED) on the morning of 11/16/18 for suicide ideation.
Findings included:
Patient #1 presented in the hospital's ED on 11/16/18 for a chief complaint of suicide ideation. Patient #1's name was incorrectly entered in the computer system. The second syllable of the patient's name was spelled "TEN." This should have been "TAN" according to the patient's driver's license that was scanned in the hospital's computer system on 03/07/18. All entries in the medical record and laboratory specimens had misspelled Patient #1's name.
During an interview on 01/07/19 at 1:00 PM in a conference room, Personnel #5 was informed of the above findings and she confirmed the findings.
