Based on findings from observation and staff interview, several rooms containing combustible items were not protected in accordance with NFPA 101 (2000) section 8.4 titled Special Hazard Protection. Section 8.4 requires rooms containing combustibles be protected by a 1-hour fire barrier, an automatic sprinkler system, or both depending on the degree of hazard posed by the combustibles.

Findings include:

-per observations at 2:45 p.m., 2:46 p.m., 2:50 p.m., 3:10 p.m., and 3:25 p.m. on 6/26/14, the following rooms were found to contain combustibles that increased their degree of hazard greater than that of the general occupancy of the building: A123 (clean linen storage and wood ceiling), A124 (dirty linen/general storage and wood ceiling), A125 (general storage and wood ceiling), laboratory storage (contained general storage items such as plastic containers, vials, boxes of records, and laboratory coats), and HIM (medical records storage).
-per interview with the Physical Plant Manager who was present at each of these locations at the listed times, he/she stated that the degree of hazard was greater than that of the general occupancy of the building.
