Based on interview and record review, it was determined the facility staff failed to document in the patient ' s medical record that a list of home health providers was presented to the patient or to the patient ' s representative for two (2) out of five (5) records sampled.
The findings include:
On September 6, 2022, the surveyor conducted a medical records review for Patients #1-5 with Staff Members #2 and #3. During the review of the medical record for Patient #2 with a discharge date of August 9, 2022, a home health preference sheet for the patient could not be located. During a review of the medical record for Patient #3 with a discharge date of July 17, 2022, a home health preference sheet for the patient could not be located. Staff Member #2 and #3 confirmed the medical record did not contain this document.
On September 6, 2022 at 10:03 a.m., the surveyor conducted an interview with Staff Member #2. Staff Member #2 advised that patients who require home health services are required to be presented with a list of available providers. The patient then makes a decision of who they would like to use for home health services from that list and they sign a patient choice letter.
A review of the policy titled "Interdisciplinary Care Coordination and Discharge Planning" states in part:
"Home Health Referrals........
When the patient makes a choice, the Patient Informed Choice Letter is completed, and a copy placed on the patient chart."
