Based upon observation and staff interview, the facility fails to assure that smoke barriers provide at least a one half hour fire rating. The deficient practice would not prevent the passage of fire or smoke to other areas of the building, affecting 2 of 4 smoke zones. The facility has a capacity of 25 in the LTCU and 13 in the Hospital with a census of 23 and 3, respectively, at the time of the survey.
Findings include:
During the tour conducted on 10/13/2010, between 1:00 P.M. and 3:30 p.m., the following is observed:
-- 1. Smoke barrier wall separating the LTCU and Hospital has unsealed penetrations around wires, cables and conduit that pass through it.
Staff M was present and acknowledged that the gaps are not sealed and would not resist the passage of smoke
NFPA Standard: Requires smoke walls to have a fire resistance rating of at least a half hour and to be continuous from floor to roof deck and from outside wall to outside wall. 2000 NFPA 101, 19.3.7.3

The deficient practice would not prevent the passage of fire or smoke to other areas of the building, affecting 1 of 4 smoke zones. The facility has a capacity of 25 in the LTCU and 13 in the Hospital with a census of 23 and 3, respectively, at the time of the survey.
FINDINGS INCLUDE:
During the tour conducted on 10/13/2010, between 1:00 p.m. and 3:30 p.m., it is observed that Resident Rooms 207 and 209 have been converted to storage rooms. Doors are not provided with closers.
Staff M was present, aware of the findings, and stated he would install closers on both doors.
NFPA Standard: Hazardous areas shall be safeguarded by a fire barrier of one-hour fire resistance rating or provided with an automatic sprinkler system and doors shall have self-closing devices and positive latches. 2000 NFPA 101, 19.3.2.1 and 2000 NFPA 101, 8.4.1
