Based on a review of policy, facility documents and interview with staff (EMP) it was determined the facility failed to ensure that, for all patients who presented to the emergency department (ED), their information was entered on the central log.
Findings include:
A review of Holy Spirit Hospital policy entitled "Triage" last reviewed June 2013 revealed, " ... 1. Patient arrives 2. Patient speaks with Greeter or Registration Clerk 3. The following information is obtained: Patient name, Date of Birth, and Chief Complaint and the patient is signed in or Quick Registered. ... "
A review on August 7, 2013, of the August 5, 2013, ED log revealed the name of complainant's husband was not listed.
A review of hospital video-tape on August 8, 2013, at 2:40 PM revealed that the complainant and her husband entered the ambulatory emergency department entrance and proceeded to the registration desk on August 5, 2013, at 5:08 PM. The complainant and her husband departed the hospital at 5:12 PM.
An interview conducted on August 8, 2013, at 3:10 PM with EMP3 confirmed that she had contact with the complainant and her husband and had not entered the husband's patient information in the log.
