Based on visual observation the facility failed to assure that all doors within an exit passageway had hardware as approved means. When doors to stairwells, smoke barriers, horizontal exits or hazardous areas doors are not maintained to its approved means it provides an opportunity to allow fire and/or smoke to flow freely throughout the facility.

Findings:

During the facility tour, between the hours of 9:00a.m. and 2:00p.m. it was observed the door located in the Laundry room did not have the proper fire rated hardware, device or knob.

Interview with manager revealed the facility was not aware that the door did not have the proper hardware to maintain the approved means.

Based on visual observation the facility is lacking a generator remote manual stop station of a type to prevent inadvertent or unintentional operation located outside the room housing the prime mover, where so installed, or elsewhere on the premises where the prime mover is located outside the building as per NFPA 110: 5.6.5.6 The location of manual stop should be readily accessible free of any obstructions on either a wall or bracket at a distance of a minimum of 25 feet from the generator prime mover. NFPA 110:5.6.5.6.1 The remote manual stop station shall be labeled. NFPA 110:5.6.5.2 (4) A individual alarm indicator to annunciate any of the conditions listed in Table 5.6.5.2.

During the facility tour, between the hours of 10:00am to 2:00pm the facility has two generators with the manual stop on the outside generators. The facility has one generatore located within the facility (unkown age of installation) at time of inpsecton. Thisgenerator age of installation shall be provided or install the remote button on the outside of the building.

Interview with administraor revealed the facility was not aware that the emergency generator is required to have a remotely located manual stop.
