Based on staff interview, review of documents, and direct observation it was determined the hospital failed to maintain the physical environment to ensure an acceptable level of quality. Findings include:
On 11/18/10 at 1115 patient room #'s 531, 536, and 538 were observed. The baseboard in the bathrooms of those rooms were observed to be pulled away from the wall/floor areas near the entry to the showers, resulting in cracks/gaps and visibly dirty and uncleanable surfaces. The RN Supervisor and Environmental Services Manager, who were present during these observations, acknowledged that these surfaces were uncleanable and were in disrepair.
