Facility failed to maintain doors in accordance with the 2000 edition of the Life Safety Code, NFPA National Fire Protection Association 101. Findings included the following:
Surveyor observed between 1 and 2:30 pm on 8/30/11 that one of two doors to corrior from laboratory and one of one doors from "Phelbotomy" were held open to corrior with wedge or kick- hold open device.
This is an impedient to closing the doors.

Facility failed to maintain hazardous areas in accordance with the 2000 edition of the Life Safety Code, NFPA 101. Findings included the following:
Surveyor observed between 2:00 and 2:30 on 8/30/11 that one of one Linen storage room, a room approximately 6 feet x 12 feet = 72 sq ft was not protected with Automatic sprinkler or separation on 2 walls and door closer was disabled.

Facility failed to maintain one of five exits in accordance with the 2000 edition of the Life Safety Code, NFPA 101. Findings included the following:
Surveyor observed between 2:00 and 2:30 pm on 8/30/11 that double doors leading to outside from nursing corridor were secured with iron bar and door latch was held open. This is not accessible to egress from the facility.

Facility failed to maintain electrical receptacles in the patient care areas in accordance with the 1999 edition of the Standard for Health Care Facilities, NFPA 99. Findings included:
Surveyor observed between 1:30 and 2:00 pm on 8/30/11that facility records did not include grounding system tests in patient care areas. Receptacles shall be tested in Patient Care areas as follows:
Phyisical integrity of each receptacle (visual)
Continuity of the grounding of each receptacle (test)
Correct polarity of hot and neutral connections in each receptacle (test)
The retention force of the grounding blade shal be not less than 4 ox (test)
Testing intervals:
General care areas tested annually
Critical care areas tested semi-annually
wet location tested annually.
