Based on observation and interview, it was determined the facility did not perform monthly testing for 3 of 3 (Operating Room #1, #3, and #4) line isolation monitors in accordance with NFPA 99, Section 3-3.3.4.2 (b) (1999 edition). Failure to test line isolation panels monthly had the potential to affect the health and safety of all patients in the surgical areas served by the monitors because proper functioning of the monitors to prevent electrical shock was not monitored and assured. The failed practice had the potential to affect all surgical patients and staff. The findings follow:
A. While on tour of the facility on 10/27/2015 at 1305, it was observed there was no sticker certifying monthly testing had been performed on the line isolation monitors in Operating Room #1, #3, and #4.
B. During an interview with the Maintenance Technician on 10/28/2015 at 1530, it was verified monthly testing was not performed on any of the line isolation monitors.
