Based on review of dietary audits and staff interview, the facility failed to ensure hand hygiene practices in the kitchen and cafeteria were corrected. The findings were:
According to the survey dated 8/12/14 when the deficient practice was identified, the facility responded with a plan of correction that showed the practice would be corrected by 8/15/14. The following concerns were identified:
a. Review of the September and October 2014 audits related to hand washing and disposable glove use showed 3 of the 5 audits conducted identified employees were not in compliance with hand washing/glove use. There were 3 employees who did not follow proper procedure and the date on these audits was 10/14/14.
b. Interview with the certified dietary manager on 11/6/14 at 10:38 AM verified although formal education was provided she continued to see non compliance with proper glove use. She verified each time an employee was identified not using acceptable techniques, they were talked to at that time. However, she stated there remained "consistent" issues with not changing gloves.
