Based on review of two (Nurse #1 & Nurse #2) performance evaluation, the hospital failed to ensure that Nurse #1 and Nurse #2's performance evaluations were completed by a supervising nurse.
Findings include:
Four nursing personnel files were reviewed. Nurse #1 and Nurse #2's performance evaluations were completed by a Radiology Technologist/Manager, not by a member of the Nursing Department. Only a member of the Nursing Department is qualified to evaluate the nurses performance in providing nursing care to patients in the Department of Radiology.
